Dr. Kiran C. Patel
Founder & Chairman
Recently named Floridian of the Year 2018 by Florida Trend Magazine, since the early 1980s, Dr. Kiran C. Patel has been taking the entire state of Florida by storm. Dr. Patel is a prominent leader among the Indian community; among the Carrollwood community where he has lived for over 30 years; and among the whole Tampa Bay community.
Kiran Patel was born in Zambia, Africa to Asian-Indian parents. He was educated under the British Educational System in Zambia and then got his diplomas from Cambridge University and The University of London. He then went to study medicine at Gujarat University in India and did his Internship in Africa. Dr. Patel did his residency in Internal Medicine in New Jersey in 1980. He completed a fellowship in the cardiology program affiliated with the Columbia University of New York in 1982.
Dr. Patel moved to Tampa, Florida in 1982 and began his practice in cardiology. He was very successful and became a distinguished cardiologist in that area. He developed a physician practice management company and expanded throughout the Tampa Bay area, diverging into 14 practices including Family Medicine, Internal Medicine, Pediatrics and Cardiology.
Dr. Patel was also in partnership with many point-of-service locations to form a multi-specialty network. This network helped patients access most medical services conveniently. He has actively engaged himself in managed care contracts and it has expanded considerably, providing care now for more than 80,000 patients. Apart from this, he has developed good associations with several HMOs and hospitals.
His success in managed care contracts led a group of doctors to seek his services to help them with an HMO in New Port Richey, Florida. Dr. Patel took up the project after discussing a predetermined purchase option of the company. It was called the Well Care HMO, Inc. (Well Care). In 1992, Dr. Kiran Patel, along with Rupesh Shah, CEO, and Pradip Patel, President, started a Medicaid managed care company. This company became the largest Medicaid provider in the state of Florida.
Dr. Patel also helped to bring around the struggling HMO, WellCare of New York and Connecticut. He worked with more than 95 hospitals and a few hundred physicians in settling past due medical claims.
Between 1995 and 2002, Dr. Kiran Patel built the business into a billion-dollar company, providing services to more than 450,000 members, employing more than 1,200 employees and operating in Florida, New York and Connecticut.

Mr. Panara is currently CEO and President of KARE PharmTech, LLC. He has over 18 years of experience in the healthcare industry holding executive positions. He served as Executive Vice President of Freedom Health, Optimum Health, and America’s 1st Choice Holding to name a few. Mital joined Freedom Health at the company‘s inception and helped to achieve the seventh fastest growing Private Company in USA per INC 500. He created and managed the SNP (Special Needs Plan) for Freedom and during his tenure it became the 2nd largest SNP provider in the USA.
In addition, Mr. Panara served as the Executive Committee member for a local  Chamber of Commerce and part-time Professor at West Bay Career University.
He graduated Magna Cum Laude with an MBA in Business/Finance and also served as President of Honor Society and President of Learning Society.
A proficient and well-known public speaker, Mr. Panara has spoken at various conferences held by leading organizers in the healthcare industry, including World Congress, Financial Research, RISE Group, World Research Group, Opal Group etc. on topics such as Medicare, Health Care Revenue Management and Compliance.
Michael Muchnicki

Michael is a senior leader with over 25 years of CEO experience, driving growth and turnaround strategies for health organizations from start-ups to multi-billion-dollar enterprises. Specializing in Commercial, Medicare, and Medicaid markets, he has developed innovative insurance products and value-based care solutions. Michael’s career includes CEO roles at UnitedHealth Group, Sterling Insurance, and other major national health plans across the U.S. He has advised prominent health systems and private equity firms, focusing on integrated delivery models and MSO strategies. Michael holds a business degree from the University of Kentucky and completed the Executive Program at the Wharton School of Business. He has also served on multiple boards, including AHIP and the American Red Cross.

Mr. Coots is a seasoned executive, business strategist, and Amazon best-selling author with a proven track record in sales, marketing, and business growth. With 24 years in the insurance industry, he has been instrumental in scaling agencies and brokerages, including expanding a sales force from 400 to over 1,500+ agents in just three years—quadrupling production while developing key partnerships and innovative training programs. ​
​As an EVP at KARE Health Partners, Mr. Coots leverages his expertise to solve problems and drive growth across all lines of business in KARE. Dr. Kumaravel Perumalsamy is a physician, healthcare entrepreneur, and technology innovator focused on building scalable, AI-enabled care delivery models that improve access, coordination, and outcomes across healthcare systems.
An interventional gastroenterologist based in California, Dr. Perumalsamy combines clinical expertise with a strong background in healthcare operations, digital health, and value-based care transformation. Throughout his career, he has worked at the intersection of medicine, technology, and entrepreneurship to address systemic challenges in healthcare delivery.
Dr. Perumalsamy is a co-founder and leader behind several healthcare initiatives focused on modernizing care models through artificial intelligence, digital platforms, and integrated clinical services. He plays a key leadership role in developing physician-supervised, AI-driven healthcare platforms designed to support patients with chronic conditions, obesity, metabolic disease, and other complex health needs.
He is actively involved in building innovative care delivery solutions that integrate providers, hospitals, health systems, and payers into a unified digital ecosystem. These platforms leverage advanced analytics, automation, and interoperability to improve care coordination, optimize treatment pathways, and reduce administrative burden for healthcare providers.
Dr. Perumalsamy is also deeply engaged in advancing digital health infrastructure that supports preventive care, remote patient monitoring, and longitudinal patient engagement—particularly for underserved and rural populations. His work focuses on aligning clinical innovation with emerging healthcare policies and value-based reimbursement models.
Beyond his clinical and entrepreneurial work, Dr. Perumalsamy collaborates with healthcare leaders, technology companies, and investors to accelerate the development of next-generation healthcare platforms that combine medical expertise with artificial intelligence and data science.

Caleb DesRosiers

Caleb is a Commercial Operations & Policy Executive with extensive experience in health policy and strategic market access. He began his career drafting healthcare legislation in the Massachusetts Legislature before serving as a Special Assistant at the Centers for Medicare and Medicaid Services (CMS) in Washington, DC, advising on payment policy and regulatory affairs. After CMS, Caleb held roles at Pfizer and Roche, supporting cross-functional payment policy and managed care initiatives. Since 2010, he has provided consulting to life science and healthcare technology companies and is currently SVP of Commercial Operations & Policy at COPILOT and a Senior Principal at Valuate Healthcare Consultancy. Caleb holds a B.A., MPA, and law degree from Suffolk University and serves on multiple health-related boards, including Daxor Corporation and the Health Alliance Plan of Michigan.
